Thanks for visiting our site. Please feel free to fill out the below form. If you have more than 1 child you MUST fill out more than 1 form per child. Be sure to include all the requested info. If anything is missing on your application it will not be processed. The deadline for applications is November 25, 2020, at noon. Anything after this time will not be accepted. PLEASE DO NOT CALL as we are out of the office.
Once your child(ren)’s name has been added to the list you will be contacted via email for info regarding pickup. Please watch your emails.
Be sure to bring children and their documentation confirming their relationship to you as well as their social and your driver’s license with you. While we would love to help everyone that contacts us, we have limited resources.
Finally, we only have 1 day to distribute toys. Be sure to track your email and meet us at the specified time as we will NOT be able to hold toys. Thanks for your cooperation and we look forward to serving you.
- It’s not necessary to call, please simply fill out the application for each of your children to ensure they are all added to the list.

- Also, if you call and leave a message this does not ensure your child’s name will be added to the list. We will not check messages.
